Getting Started

1. make an inquiry

For inquiries, please phone us, fill out our inquiry form or pop into our offices for a chat. We will tell you all about Sapphire healthcare, how we work and what we stand for. You can meet our team and ask all your questions. We will also send you a full company brochure, costs of our services and further literature so that you have all the information you need.



2. assessment

once you are happy with the information we have provided you about Sapphire healthcare and would like to move forward to the next stage, we will arrange a needs assessment meeting at a convenient time for you and any family members or representatives that you would like to be present with you. This meeting should take place at the clients home as that is where the care is to be provided. The assessment meeting is our opportunity to collectively talk about you and your needs, discuss the care service options available and formulate a care package tailored to meet your needs. this is also an opportunity to get to know each other as well as assessing any potential risks within your home environment either for yourself or our care staff.



3. proposed care plan

once we are fully aware of your needs and preferences, and have received any relevant information if required, from any other parties or agencies involved in caring for you we will then proceed to produce a proposal document of the care package that is most appropriate for you. This proposal document will align the full extent of the service we can offer, how and when we will deliver the care and a full breakdown of the costs and contractual terms involved.

4. agree care plan

upon your satisfaction with a proposed care plan and agreeing to proceed with working with sapphire healthcare, we will all sign and agree the contract which will detail our obligations to each other. we will both keep a copy of our records and contracts. Our care plan will also include how to change any aspects of the care service, the procedures to follow if there are any issues as well as full details of every aspect of the care service we will be delivering.



5. meet your carer

during the first few care visits, we will always accompany our care worker with a senior care manager. This allows us the opportunity of assessing how are assigned care worker is bonding with you and if there are any concerns, this can easily be discussed with the senior manager. Once you, our senior care manager and care worker are satisfied with all aspects of working directly with each other, Only then do we allow the care worker to work alone with the client.



6. ongoing assessments and support

From our quarterly satisfaction surveys to our continuous care worker appraisals, we ensure every aspect of our service is consistently delivered to the highest standard.
